Mood/Personality Disorders and Other Mental Health Services


Case Management and Care Coordination Services (In-Home or Remote):

Mood and personality disorders, as well as other mental health issues, require long-term clinical treatment often with several providers involved.  To ensure continuity of care, clinical providers should receive regular updates and collaborate with the creation of the treatment plan. OPG’s Case Management and Care Coordination Services can facilitate an open dialogue among therapists, psychiatrists, DBT coaches, therapy group leaders and other treatment team members.

Services include:

  • Compilation of medical records from past inpatient and outpatient providers
  • Assistance in forming a comprehensive treatment team in a client’s local area (if one does not exist) that addresses medical, nutritional, and psychological needs
  • Coordination with assigned clinicians to create a case management plan with short and long-term goals, and tracking and notifications of progress over time
  • Assigned case manager to provide guidance and support to client through regular in-person or remote meetings, and frequent phone contact
  • Weekly communication with members of the outpatient treatment team
  • Coordinating treatment team meetings
  • Interface with insurance companies
  • Location of vocational pursuits (e.g., employment, educational or volunteer opportunities)

In-Home Life Skills Coaching

OPG offers in home life skills coaching to individuals struggling with a Mood Disorders, Personality Disorders or other mental health issue. Our Life Skills coaches will meet with you to help identify areas of need and develop an individualized plan to address them. Our coaches work intensively with individuals, while coordinating closely with their treatment teams.

Services include:

  • Environmental and Community Integration (e.g., organizing home environment, connecting clients with local resources based on areas of interest, attending social events with client)
  • Coping and Interpersonal Skill Building (e.g., reinforcement of DBT skills in coordination with treatment team, behavioral rehearsal of coping strategies, modeling of social norms and healthy relationship boundaries
  • Yoga and Meditation/Mindfulness Training (e.g., meditation guidance, mindfulness education, exploring and vetting local meditation/yoga resources)
  • Stress and Time Management (e.g., creating behavioral schedules, supervising difficult tasks, teaching stress management strategies, providing prompts for task completion)
  • Vocational Coaching (e.g., exploring career/academic interests, vocational testing, resume building, job application support)
  • Academic Support (e.g., researching academic options, coordinating credit transfers, study skill support)
  • Physical Fitness (e.g., locating specific fitness centers, engaging in rock climbing, working out, hiking and biking with clients)
  • Nutritional Guidance (e.g., creating balanced grocery list and budgeting, accompanying clients to the grocery store, cooking meals and supporting clients in making healthy meal choices in restaurants)
  • Budgeting and Financial Planning (e.g., creating and maintaining budget and implementing tools to track expenses)
  • Self-Help Meeting Navigation (e.g., finding support groups for a client in his or her local area for clients with co-occurring addiction issues)
  • Transportation (e.g., driving clients to local appointments, support in navigating public transportation)
  • Medication Monitoring (e.g., assisting with pill cassette organization and checks, providing reminder prompts as needed)
